Help at Home is a person-centred service that aims to help people return home from hospital in a safe and timely manner, prevent an admission or readmission to hospital and make connections in their community. Their aim is to ensure those we care for have an enjoyable life.

Care Network’s Help at Home is a service that offers free practical and emotional support in the home. The service is for any adult who has come out of hospital or is experiencing an illness or crisis at home. Anyone, including individuals themselves, concerned families, hospital or community-based professionals and other organisations can make a referral for help.

This is a free service.

 

Other services offered by Care Network:

  • Community Navigator – information and support to access local services and activities to improve personal circumstances and wellbeing.
  • Wellbeing Support – strength-based tailored one-to-one support for people feeling low or anxious to help them develop coping strategies, make personal connections, and build resilience.

Care Network are looking for volunteers to join their team to help people living in Cambridgeshire and Peterborough to remain independent and maintain social connection.
